Friendly Reminders

• Provide your most current GHP ID card to your health care providers at each visit.

For additional or replacement ID cards, contact our Benefits Resource Specialists.

• Over-age 18 participants require a separate login account. Refer to the Online Claims

System Quick Reference Guide under the Documents tab for additional

information and instructions.

• GHP requires precertification for certain services and procedures.

For inpatient hospital admissions, certain outpatient services, such as laminectomy

and hysterectomy, and medical equipment (this is not an all-inclusive list) providers

should contact United Clinical Services at (800) 708-8940.

For precertification of outpatient advanced imaging such as CT scan, MRI and PET

scans providers should contact eviCore at 866-581-5253 or initiate a request by

visiting www.evicore.com.
