Fiber Demand Survey Official Contest Rules

NO PURCHASE OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN THIS PROMOTION.  A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING.  THIS PROMOTION IS VOID WHERE PROHIBITED BY LAW.

1. SPONSOR.  The National Telecommunications Cooperative Association d/b/a NTCA–The Rural Broadband Association (“Sponsor”) will sponsor a promotion ("Promotion”) as part of the Fiber Demand Survey (“Survey”) sent in January 2022.
2. ELIGIBILITY. The Promotion is open to individuals who: (1) complete and submit the Survey, answering every question, during the eligibility period beginning January 5, 2022, and ending January 26, 2022 (“Registrants”); (2) are employees or representatives authorized for purposes of completing the Survey of an Incumbent Local Exchange Carrier (ILEC) member of NTCA–The Rural Broadband Association; (3) are legal residents of a state or the District of Columbia in the United States of America; and (4) are at least 18 years of age at the time of their entering this Promotion. Only one person per ILEC member of NTCA–The Rural Broadband Association may win a prize.  Employees and their immediate family members and members of the same household of Sponsor are not eligible to participate in this promotion.
3. PROMOTION AND PRIZES.   This Promotion will consist of a drawing (“Drawing”) with 45 winners.  The Drawing will take place on January 27, 2022.  Forty-five (45) prizes will be awarded in this Promotion – this is, one prize per each Registrant Drawing.  Odds of winning depend on the number of eligible Registrants.  

All prizes are non-assignable and non-transferable, and are not redeemable, exchangeable or substitutable for cash.  All taxes and other expenses associated with a prize are each winner’s sole responsibility.  Sponsor reserves the right to substitute a prize of equal, or greater value.

4. HOW TO ENTER. Employees or authorized representatives of companies that are ILEC members of NTCA–The Rural Broadband Association may complete a Fiber Demand Survey.  The Survey may continue after January 26, 2022, but only eligible individuals who fully complete and submit the survey by January 26, 2022, will be entered into the Drawing.
5. SELECTION OF WINNER.  The winners of the Drawing will be notified by email.
6. TO CLAIM A PRIZE.  The prizes are Visa gift cards that will be sent via U.S. mail to winners of the Drawing. To claim the prize, within fourteen (14) days of receiving notification of winning in the Drawing, a winner must provide a physical mailing address for receipt of the prize.  Sponsor is not required to deliver the prize if such information is not received within the timeframe stated above.
7. DELIVERY OF PRIZE. Within ten (10) days winners providing the requisite information, Sponsor will initiate delivery of the prize. Sponsor is not responsible for any lost shipment or other losses or delays associated with transmittal.
8. PERSONAL INFORMATION.  By law, Sponsor is required to release a list of the winners if requested.  If such request occurs, Sponsor agrees to notify winners prior to the release of the list of winners.  Also, personal information submitted in all entries will be subject to Sponsor’s Privacy Policy located at https://www.ntca.org/ntca-website-privacy-policy.
9. GENERAL.  This Promotion and these Official Rules will be governed by and construed in accordance with the laws of the Commonwealth of Virginia.  Sponsor and affiliates will not be responsible or liable for: (i) any late, lost, misrouted, garbled or distorted or damaged transmissions or entries; (ii) telephone, electronic, hardware, software, network, Internet, or other computer- or communications-related malfunctions or failures; (iii) any printing or typographical errors in any materials associated with this Promotion.  Sponsor reserves the right, at its sole discretion to modify, cancel, terminate, extend or suspend this Promotion should any virus, bug, technical failures, unauthorized human intervention or other causes beyond Sponsor’s control corrupt or affect the administration, security, fairness or proper conduct of this Promotion.  Sponsor reserves the right, at its sole discretion, to disqualify any entrant if his or her fraud or misconduct affects the integrity of this Promotion.  By participating in this Promotion, each entrant accepts the terms and conditions stated in these Official Rules, agrees to be bound by the decisions of Sponsor, and warrants that they are eligible to participate in this Promotion.
10. WINNERS LIST. For a list of winners in this Promotion, send a Self-Addressed, Stamped Envelope to:  NTCA/ Fiber Demand Survey, NTCA–The Rural Broadband Association, 4121 Wilson Blvd., Arlington, VA 22203.  Winners’ names will not be made available until after all the winners in this Promotion have been determined and notified.
